Diabetes Book & Learn
Bexley Health Neighbourhood Care CIC (BHNC) are currently commissioned to deliver the Diabetes Book and Learn call centre service, by South East London Integrated Care Board (SELICB) and South West London Integrated Care Board (SWLICB) jointly.
The service provides a call centre team to support patients to book onto diabetes structured education (DSE) by offering them a wide choice across all 12 boroughs, which allows more course availability and options for our patients. This service will work in partnership with the DSE providers across the 12 boroughs of South London and the digital platform provider, Priority Digital Health (PDH) to deliver this service. We will receive referrals into the platform from GP referral as well as patient self referral.
The aim of the call centre will be to engage, encourage and support patients to book onto an education session, that is most suited to their preference, our team will use their skills in motivational interviewing and knowledge of the available education programmes to:Engage : with patients on the phone to understand their individual needs and requirements to attending structured education. Encourage: the patients of the importance of attending education by providing the patients with all the necessary information to make their attendance as easy as possible and explore the benefits to their long term health by attending. Support: supporting patients by providing a listening and understand all the barriers to education and guide patients to make the right choice, including supporting with the on-boarding and access to the course.
