As part of our continued efforts within Clocktower Primary Care Network to improve patient wellbeing and reduce the risk of heart disease within our local GP surgeries.

 

As part of those efforts, patients will be invited for a face- to- face heart health check-up at our new guard your heart community hub at Bellegrove surgery.  These are run on Thursday mornings, and patients will meet with a specialist pharmacist.

When patients are due for a review of their medication to make sure that the patients treatment is the best  this review will be carried out at the same time.

​

Patients will require a blood test 2 to 4 weeks prior to the appointment.

​

The  appointment will last for approximately half an hour, and it will include:

• Checking  blood pressure and heart rate

• Talking through  blood test results

• Reviewing  medication through a discussion of options 

• A brief discussion about  lifestyle and wellbeing

 

Following this appointment, the service will  arrange regular heart health check-ups  and there will also be the opportunity to join a patient-to-patient group that discusses wellbeing and heart health
