BHNC operate a house visiting service for patients with specific long term conditions who are housebound and in need of regular reviews.   Our team of trained HCA’s and phlebotomists, will visit you at home at a pre booked appointment, to collect all the required tests for your long term condition.  This will likely include blood pressure, weight and waist checks (where possible), and may in some instances include blood testing and urine collections.  The current conditions that we provide this long term condition annual review service for are:

• Type 1 Diabetes

• Type 2 Diabetes

• Asthma

• Chronic Obstructive Pulmonary Disease (COPD)

• Coronary Heart Disease (CHD)

• Dementia

 

Our team will receive referrals direct from the GP of the patient and we will then contact the patient to book an appointment.  This service allows us to support GP practices by collecting the initial tests in the home of the patient, the results are then reported back to the GP for them to follow up with any additional visits if required.
