Primary Care Support in Bexley

Bexley Health Neighbourhood Care (BHNC) is proud to support all 19 GP practices across Bexley as well as support the four Primary Care Networks (PCNs), working together to improve health outcomes, strengthen primary care services and enhance the experience of patients across the borough.

Our role is to provide practical, strategic and operational support that enables practices and PCNs to focus on what matters most – delivering high-quality care for their patients. By working collaboratively at scale, we help practices respond to increasing demand, national NHS priorities and the changing needs of local communities.

How We Support Practice

BHNC works alongside practices and PCNs to provide:

PCN business management and governance support
Project and programme management
Quality improvement and service transformation
Workforce planning, development and supporting with recruitment & retention
Financial and funding support
Data, analytics and performance monitoring
Digital transformation and access improvement
Clinical programme delivery and coordination
Developing successful business cases and supporting in their delivery.
Support with NHS contracts, assurance and reporting requirements

Our experienced team works closely with practice managers, clinicians, PCN Clinical Directors and wider system partners to help practices navigate an increasingly complex healthcare environment while maintaining a focus on patient care.

Working Through Primary Care Networks

Primary Care Networks bring neighbouring GP practices together to deliver more coordinated, joined-up care for local populations. In Bexley, the four PCNs work collaboratively to share expertise, resources and best practice, improving access to services and reducing health inequalities across the borough.
BHNC supports each PCN by providing the infrastructure, expertise and operational delivery required to turn strategic ambitions into real improvements for patients, practices and communities.

Through strong partnerships with GP practices, PCNs, the South East London Integrated Care System, local providers and community organisations, BHNC helps deliver innovative programmes that improve prevention, access, long-term condition management and neighbourhood-based care. Together, we are building a stronger, more sustainable primary care system for Bexley, one that supports our workforce, improves patient outcomes and helps local people live healthier lives.

EA Clinics

BHNC support the GP practices of Bexley to provide the primary care extended access service. We provide GP and nursing appointments to registered Bexley patients, of an evening between 6.30pm - 8pm and on a Saturday 9am - 5pm. These appointments are bookable by your GP service.

Phlebotomy Service

BHNC support the practices of Bexley by providing a phlebotomy service in a number of practices throughout the borough. This service allows patients to access phlebotomy closer to home. Practices are able to book their own patients direct into the BHNC phlebotomy clinics.

Home Visiting Services

BHNC support the GPs of Bexley by providing a long term conditions annual review service. Our team of trained HCA's attend the homes of patients to undertake the data collection part of the long term conditions annual review and take blood if required. Practices can refer patients direct to the BHNC team.

INT (Integrated Neighbourhood Team)

We support the development of Integrated Neighbourhood Teams across Bexley, bringing together health, social care, voluntary sector and community services to provide joined-up care for local residents. By working closely with GP practices and partner organisations, we help coordinate services, improve access to support, and ensure people receive the right care, at the right time, closer to home.
