Preventative Services to Help You Stay Well
Bexley Health Neighbourhood Care supports a range of preventative services designed to help local people stay healthier for longer. Our focus is on early identification, proactive support and helping people make informed choices that reduce the risk of long-term complications.
Through programmes such as NHS Health Checks, X-PERT Diabetes, Stop Smoking support and neighbourhood-based care, BHNC helps patients take positive steps to improve their health, manage existing conditions and access the right support at the right time.

BHNC works with practices, Primary Care Networks and local partners to provide preventative services that support patients across Bexley, including:

Diabetes Book & Learn
Our X-PERT Diabetes service delivers the six week structured X-PERT programme to patients living with type 1 or type 2 diabetes, to improve their long term health outcomes and management of their condition. These courses are available online and at venues across Bexley borough. Patients can self refer or be referred by a clinician.

Stop Smoking Service
We deliver the Stop Smoking Service for Bexley with three advisors working to support residents of Bexley who want to stop smoking. Clinics are available over the telephone and at some face to face venues across the borough. Patients can self refer or be referred by a clinician.

NHS Health Checks
We support the practices of Bexley to deliver the NHS Health Checks programme, that help identify patients at risk of cardiovascular disease and diabetes. Patients aged 40 to 74 who do not have certain conditions will be invited to receive a health check. After a health check, patients can be referred and signposted to programmes that can support their long term health.

INT (Integrated Neighbourhood Team)
We support the development of Integrated Neighbourhood Teams across Bexley, bringing together health, social care, voluntary sector and community services to provide joined-up care for local residents. By working closely with GP practices and partner organisations, we help coordinate services, improve access to support, and ensure people receive the right care, at the right time, closer to home.
